Specialty drug costs have continued to push the boundaries in terms of complexity and affordability. The complicated landscape must consider many variables, such as multiple dispensing channels, dual payment methods, and site of care. This report takes a detailed look into what plan sponsors are facing as they make decisions relative to specialty drug benefit plan design.

Alternative funding continues to be a hot topic as plans explore strategies to help offset the high cost of specialty drugs.

"As plans continue to seek ways to offset specialty drug costs, the market is divided as to whether alternative funding is a positive option or a detriment," commented Michael Lonergan, RPh, PSG President. "Affordability of specialty drugs for plans and members is driving this interest, while there is a concern about sustainability in these models."

Integrated pharmacy and medical claims data for managing specialty drugs remains essential.

"Total cost of care is a key concern, and payers rely on reporting that details how well specialty drugs are performing through the insights of integrated data. However, the challenge is in tracking clinical efficacy," observed Morgan Lee, PhD, MPH, CPH, Senior Director, Research and Strategy.

Affordability of cell and gene therapies will likely be a significant payer challenge in coming years.

"As the cost of specialty drugs continues to increase, some health plans and fewer employers have explored the use of value-based contracting (VBC), a performance-based reimbursement agreement. Rare disease treatments that are being targeted by cell and gene therapies will drive future spending," noted Renee Rayburg, RPh, Vice President, Specialty at PSG. "VBCs represent an effort into finding strategies for keeping costly specialty drugs accessible and affordable."

KEY FINDINGS FROM THE REPORT INCLUDE:

* Gene Therapies: While just 24% of respondents have had a gene therapy patient, 81% expect affordability of gene therapies to be a moderate or major challenge in the next 2-3 years.

* Alternative Funding: 14% of employers and 7% of health plans use alternative funding models, and 14% of employers and 33% of health plans are exploring their use. However, 68% of respondents viewed these models as not at all or slightly sustainable.

* Value-based Contracting: Only 12% of respondents report using value-based contracting for specialty drugs. The need for additional evidence, difficulty agreeing on and tracking outcomes, and lack of resources and buy-in were all cited as key barriers to adoption.

* Specialty Rebates: Nearly all plans receive specialty drug rebates under the pharmacy benefit, but health plans are more likely than employers to receive rebates under the medical benefit (66% vs. 27%).

Additional topics covered in the report include increasing usage of new-to-market formulary blocks, interest in site of care programs, prior authorization tracking and reporting, and use of strategies related to biosimilars.

Comic book child star grows up to partner with writer father on Kickstarter horror titles

FORT WORTH, Texas -- Since he was 12 years old, Jake Tinsley has written and promoted his own professional comic books. Now, the 28-year-old Jake is debuting a brand new adult horror title "ALTERNATIVE NARRATIVES" which is being sold in a powerful Kickstarter, side-by-side with "THE BLONDE ASSASSIN" title written by his father Ben Tinsley. The father and son comprise the TINSLEY BOYS writing team who work out of Jason Dube's Sacramento-based Scattered Comics. They are best known for their "NIGHT OWL" character, a young shaman protecting the balance of the universe.

"ALTERNATIVE NARRATIVES" marks Jake's first comic book title without his father's writing assistance. Jake decided it was time to work alone on Alternative Narratives while his father focused completely on his other projects.

"Comic book production is a ferocious but rewarding process," Jake Tinsley said.

"It takes an incredible amount of work on everyone's part to get these comics finished - and that's just the writing and editing," Jake said in a recent interview, "After Dad and I finished THAT, our scripts then went to Mark Vuycankiat for art and Diego Vasquez for color on 'Blonde Assassin' and Alex Lopez for art and Mike Stefan for color on 'Alternative Narratives.' And insane number of top-level guest artists helped with the covers."

Phil Lockler, who promotes TINSLEY BOYS projects with Tricia Locker, said both comics are a blast to read.

"Prepare yourselves," Lockler chuckled. "They're coming in HAWT!"

In a storytelling sense, "BLONDE ASSASSIn" and "ALTERNATIVE NARRATIVE" dovetail with one another, Jake explained. There is a loose but undeniable connection between assassin sisters Surrender (The Blonde Assassin) and Monique (The Blonde Bombshell from "ALTERNATIVE NARRATIVES") who each live on alternate earths and each believe the other sister is dead. (Well, actually they ARE, in a WAY, but that's a LONG STORY.)

"Jake and I will be working together again on another horror title, 'DJINN STALKERS,' and some really cool other projects. But you'll find with this title that his writers voice is a bit different from mine. Meanwhile, Jake's getting so engaging with his writing. It's a joy to watch him continue to grow."

According to CW Service Pros – There’s A New Reason To Upgrade Your Heating and Cooling System Sooner Than 2023

LEWISVILLE, Texas -- In 2023, the HVAC industry is expected to experience major price increases in the production of new heating and cooling systems due to higher standards in SEER rating systems* taking effect in the new year. Consumers can benefit now by purchasing new HVAC systems before the close of 2022 while SEER standards remain at lower ratings, says CW Service Pros.

CW Service Pros Office Manager Karen Edmonds states, "There's a price hike coming on heating and cooling (HVAC) equipment January 2023. If your HVAC system is on the older side or you are considering replacing it soon, replacing it now could save you a lot of money."

She also stated that both by purchasing and installing a new system before the price increase and by the savings offered - "you will benefit from higher efficiency equipment."

PRICE INCREASES IN THE HVAC INDUSTRY

The U.S. Department of Energy uses SEER (seasonal energy efficiency ratio) ratings to evaluate heating and cooling equipment efficiency. The higher the SEER rating, the more efficient the system is. These ratings ensure all systems meet minimum standards of effectiveness and efficiency. They also help consumers compare and evaluate different systems and make good purchasing decisions.

The Department of Energy has revised its SEER guidelines for 2023 and a 20 SEER system is now considered a 19.2 SEER2 system. As of January 2023, Texas will be phasing out low SEER equipment to establish a higher efficiency standard in both new construction and existing homes. The minimum energy-efficiency standards are being raised for all cooling and heating systems. At the same time, a new SEER ratings system, called SEER2, will go into effect. The new SEER2 system uses different testing conditions and is a more accurate way to evaluate HVAC efficiency.

Due to the coming changes according to Karen Edmonds, "HVAC manufacturers are redesigning HVAC components to meet the new SEER2 requirements. As a result, prices are said to increase on new HVAC systems and components as of January 2023 as much as 30%."

Karen continues, "Once the new regulations go into effect, any units that don't meet the new requirements can't be sold or installed. It is against the federal law to sell or install this equipment after the New Year."

ADDITIONAL REASONS TO UPGRADE NOW

There is another great reason to upgrade now according to Karen, especially if your current HVAC system is older than 10 years or out of warranty. Replacing an old system won't just save you from having to pay more upfront; over time, it'll also save you money because new HVAC systems cost less to operate.

The State of Texas, through its oil and gas regulatory body the Texas Railroad Commission, continues to advance toward obtaining primacy from the Environmental Protection Agency for the permitting of Class VI Carbon Dioxide injection wells. This moves the State of Texas closer to a state wide and commercially viable deployment of carbon capture and sequestration.

Carbon capture and sequestration is a process that will be centered around deep saline formations underneath large land holdings in both south, west and north Texas and has the potential to provide not only long-term revenue for Texans but also create new and complex legal issues for Texas land and mineral owners.

Long term storage of carbon dioxide, pore space ownership, development of deep mineral rights beneath the storage zone, plume migration beyond lease boundaries and understanding the safety of carbon storage are all evolving subject areas affecting land and mineral owners in the State of Texas and upon which Grammer intends to continue to speak upon.
